Membership in our In-House Dental Benefits Plan is available to all patients without dental insurance. After payment of an annual fee, members will receive the following:

Dental Cleaning (D1110 or D2910)
Oral Cancer Screening
Oral Hygiene Instructions
TMJ Screening Periodontal Evaluation
Unlimited Emergency Visits:
Problem Focused Exam (D0140)
Single X-ray (If necessary) (D0220)
20% Discount Off Our Office Fees:
A treatment plan will be provided In writing for all recommended procedures displaying the discounted rate.

Individual: $249
Child: $159
No Waiting Periods.
No Restrictions on Treatment.
No Age limitations.
Coverage in Effect for One Calendar Year from Posted Date.
Coverage begins when membership has been paid In full.
Members will not be notified when year ends.
Appointments times are limited, and It is the responsibility of the member to schedule appropriate visits.
