Membership in our In-House Dental Benefits Plan is available to all patients without dental insurance. After payment of an annual fee, members will receive the following:
2 Regular Dental Check Ups:
- Exam (D0150, D0120)
- Digital X-rays (If necessary) (D0330, D0274, D0220, D0230)
- Dental Cleaning (D1110 or D2910)
- Oral Cancer Screening
- Oral Hygiene Instructions
- TMJ Screening Periodontal Evaluation
Unlimited Emergency Visits:
- Problem Focused Exam (D0140)
- Single X-ray (If necessary) (D0220)
20% Discount Off Our Office Fees:
- A treatment plan will be provided In writing for all recommended procedures displaying the discounted rate.

Coverage in Effect for One Calendar Year from Posted Date.
Coverage begins when membership has been paid In full.
